Crouch Family Medicine is the private, solo practice of Jason Crouch, D.O. The practice opened in Chandler, Oklahoma in the summer of 2007.
Unlike many medical practices in the U.S., Crouch Family Medicine is not owned or managed by any hospital or hospital group. This gives Dr. Crouch the freedom to refer you the specialist of your choice. There are many good hospitals in the area and Dr. Crouch is happy to refer you to the hospital and specialist of your choice for your specific medical need.
The clinic is a non-emergency, outpatient facility offering lab services, Digital X-Ray, and EKG. Dr. Crouch is Board Certified in Family Medicine through the American Osteopathic Board of Family Physicians. He is also certified as a Diplomate in Obesity Medicine by The American Board of Obesity Medicine, one of only 19 physicians in Oklahoma who hold this designation.

The office closes for lunch each day between 12:00pm-1:20pm.
When calling for an appointment, please indicate to the receptionist your symptoms and the nature of the consultation requested. This enables us to reserve an adequate amount of time for your particular problem(s). Every effort will be made to give you the earliest and most convenient appointment available with the provider you choose.
Because we respect your time, a sincere attempt is made to adhere to our appointment schedule. Because emergencies do arise and will complicate the best of schedules, we ask for your understanding at these times. If you will be late for, or cannot keep an appointment, please give us reasonable notice so that we may give your appointment to others in need.
Dr. Crouch and his wife Robin have been married for 20 years and have two young daughters, Tatum & Chloe.
The practice is privately owned and operated by Dr. Crouch. After medical school and a 3-year residency, he chose to return to his hometown to open the clinic due to the positive, lasting impression the people of the Chandler community have had on his life.
