First Visit

We hope to make your first visit not just an opportunity to deal with any medical concerns you may have but also a time to get acquainted with you and speak to you regarding preventive medicine specific to your needs. As a family practice, we will try to solve your current medical problem and detect or prevent other health problems.

During your first visit, we’ll cover your health history, perform medication reconciliation, discuss pertinent health concerns, and preventative care. You’ll also meet our staff, complete a few brief forms and, of course, meet Dr. Crouch or the Nurse Practitioner, JaNena Wilson. The doctor/patient relationship is important to us and the first visit is imperative to establishing proper care.

If you are interested in becoming a patient of our clinic, please call our office at 405-258-9955 to book your first appointment to establish care. We recommend you make the appointment to establish care right away before you become ill.

Make Appointment

For your first visit:

  • Please arrive 10-15 minutes early to allow time before your appointment to complete our new patient paperwork.
  • Please bring all of your current medications or a list of all current medications to your first appointment.
  • We recommend you make your new patient visit to establish care before you become ill. This makes it easier to get an appointment more quickly when you do become ill. 
  • Because of the time it takes to establish care during the first visit, we only schedule new patient appointments on Tuesday, Wednesday, and Thursday and limit the number of these appointments available each day. Plan ahead to establish care.
