When you’re hurt or sick, life just isn’t as enjoyable. Rather than just treating the symptoms, we do our best to get to the underlying cause of your issue. We advise you on the medicine and equipment you need to get back on your feet. We can often work urgent care needs into our same-day schedule for existing patients, so call us first!
We offer seasonal influenza shots for your convenience. We also offer pneumonia vaccine for our patients who qualify.
When you visit Crouch Family Medicine, you won’t need to drive elsewhere to have labs drawn or wait extended periods of time for your test results to come back from a hospital or other testing facility. That’s because we have our own on-site lab and staff trained to run most of your basic lab tests. For labs that we do have to send out, you will have your labs drawn in the comfort of our facility and typically know your results by the end of the next day. We also have on-site X-Ray and EKG available to our providers.

We provide management of hypertension (high blood pressure), diabetes, hyperlipidemia (high cholesterol), hypothyroidism, asthma, obesity, COPD, and more.
We provide diagnostic testing, diet and exercise counseling, and medication/insulin management services. Treatment includes management of other diabetic-related conditions.

Allergy issues may be a minor inconvenience for you, or they may put your life in serious danger. We can help protect you with allergy shots and other preventative care.
While we can’t cure your allergies, immunotherapy can significantly reduce your sensitivity to your allergy triggers and reduce your reaction.
Treatments vary and can include immunotherapy, antihistamines, corticosteroids, and avoiding allergens.
We will review your medical history and immunizations and perform a simple physical exam to determine if you are able to safely participate in your chosen sport. We will fill out the forms needed by your school or coach to get you in the game! As an added benefit, your forms are securely uploaded into your chart for future reference.
Get the support you need to take control of your weight and live a healthier lifestyle. We can provide expert advice on physical activity, diet, and other intervention techniques to help you look and feel your best. Our providers have specialized training and education in Obesity Management & Nutrition.
We provide a full range of Women’s Health Services including annual Well-Woman Exams. These appointments can be made with the provider of your choice. We have both male and female providers on staff so you can discuss your issues in comfort with a trusted professional.
Crouch Family Medicine can oversee all of the care that you receive, but there will be times when you require specialty services that we don’t provide. We maintain a list of trusted specialists who are experts in their areas of medicine.
We offer a streamline process for the Annual Wellness Visits to keep our Medicare patients and Medicare Advantage patients up-to-date on their preventative healthcare. This visit is an annual benefit to Medicare patients and Medicare Advantage patients with no cost to the patient. We use these appointments to give you the tools you need to keep you healthy and active. Call our clinic to schedule your AWV right away to set your year on a healthy track!
We are proud of the quality of care we provide our patients. We work hard to keep you healthy and out of the hospital and ER. For urgent (non-emergency matters) our existing patients can reach Dr Crouch or JaNena for guideance by phone 24/7/365 via our after-hours call coverage at 405-258-9955. Patients with true medical emergencies should always call 911. When you do need to visit an ER or have a hospital stay, we want to see you after you a released to help with your transition back to home. Our Hospital Follow-Up visits and ER Follow-Up visits give you the clarification and guidance you need to take care of yourself at home once you are released. We have found this time spent with patients to understand the hospital care at home orders and medication changes goes a long way to prevent re-admission. Please call us to schedule your follow-up visit and help us help you with your Transition of Care to stay healthy at home. Not able to get into the office for an in-person visit? We are able to handle a variety of your healthcare needs by a virtual visit via your smartphone. This is a convenient way to take care of those simple visits you need from your healthcare provider. Call the office to make an appointment for this and ask if your appointment can be done virtually. There is no app to download or complicated websites to navigate through. We simply send a secure link to you by text when it’s time for your appointment. Simply click on it to have your virtual visit with Dr Crouch or JaNena at your scheduled appointment time.
We provide COVID-19 testing for our existing patients. We offer both rapid antigen testing as well as rapid PCR testing. We do this curbside in our back parking area for your privacy and to ensure our indoor areas remain fever-free for the safety of all of our patients and staff.
