Jason Crouch, D.O.

Dr. Crouch is Board-Certified in Family Medicine. He was born and raised in Chandler and has been practicing for 20 years with 16 of those years in private practice. He received his medical degree from OSU College of Osteopathic Medicine in 2004 after earning his B.S. degree in Cell and Molecular Biology at Oklahoma State University. Following medical school, Dr. Crouch completed a 3 year internship and residency in Family Medicine at OSU Medical Center in Tulsa, where he was named 2007 Outstanding Resident of the Year in Family Medicine. Dr. Crouch chose the specialty of Family Medicine due to the variety of patients and conditions he could treat. He opened Crouch Family Medicine in July of 2007 where he practices primary care Family Medicine in an outpatient setting.

Dr. Crouch practices the full scope of family medicine including Adult Medicine, Pediatrics, Women’s Health, Sports Medicine, and minor emergency care. He is a member of American Osteopathic Association (AOA), the Oklahoma Osteopathic Association (OOA), and the American College of Osteopathic Family Physicians (ACOFP). He is Board Certified by the American Osteopathic Board of Family Physicians (AOBFP). He is also one of only 19 physicians in Oklahoma certified as a Diplomate of the American Board of Obesity Medicine.

When not in the office, he enjoys spending time with his spouse, Robin, and daughters, Tatum & Chloe.

JaNena Wilson, DNP, APRN, FNP-C

JaNena is a Nurse Practitioner certified in Family Practice. JaNena received her BSN from OU School of Nursing, her MSN with emphasis in Family Nurse Practitioner from University of Missouri-Kansas City, and her Doctoral Degree (DNP) from the University of Missouri-Kansas City. Before becoming a Certified Nurse Practitioner, JaNena spent 4 years as a RN in the Critical Care Unit at Deaconess Hospital in Oklahoma City. JaNena is a member of and Board Certified by the American Academy of Nurse Practitioners (AANP). She is also a member of the Oklahoma Nurse Practitioners (ONP).  JaNena is a Carney High School graduate and was happy to return to Lincoln County to practice in April 2012.

When not seeing patients, JaNena enjoys spending her free time with her family and is actively involved with her church.

JaNena has been providing exceptional care for our patients since 2012.

JaNena is currently accepting new patients of all ages by appointment.


Tracy is experienced in clinic front desk operations and serves our patients in the scheduling of appointments, reception services for patients and visitors, new patient registration, billing/account inquiries, and general customer service. She is a native of Chandler and has over 22 years of customer service experience in healthcare, insurance & banking. She has excelled in our Medical Office Coordinator position since 2010. Tracy is diligent in her duties and always working to keep the clinic running efficiently each moment of the day. She is our front desk expert, and we all know to “ask Tracy” when in doubt on clinic policy or front desk matters. We can always count on Tracy to be the first person to open our doors each morning for patients.

When not in the office, Tracy enjoys time with her family.


Deborah joined us in May of 2018. She came to us with 10 years of Medical Assistant experience in a Pediatric office. She is an M.A. and assists in all aspects of patient care.  In her M.A. position, Deborah serves our patients in blood draws, lab processing, injections, X-Rays, rooming of patients, collection of patient vitals, supply ordering, and assisting with procedures. Deborah is an integral part of the clinical team.

When Deborah is not in the clinic she’s supporting her 4 boys in their sports activities.

Susan Gardner, R.N.

Susan assists Dr. Crouch with procedures, patient care, and specialist referrals. She serves as our RN Care Manager. She has been an R.N. for 30+ years. Susan enjoys working for the clinic and has been assisting us in a part-time capacity since the Fall of 2009. Susan is a caring nursing professional who is always eager to assist us in better assisting our patients with care coordination and care management. In her RN Care Manager role, she focuses on Care Coordination including referrals and pre-authorizations, preventative care recommendations, and giving nursing guidance to our patients with complex care needs to provide them the best quality of care for optimum health outcomes.

Susan has been a vital member of our team for over 15 years. When not in the office, Susan enjoys the RV camping life and seeing the United States with her husband, Kirby.


Alyshia has served as our Medical Assistant-Phlebotomist for many years beginning her career at Crouch Family Medicine in 2013. She has also worked in our Medical Office Coordinator position at the front desk so she is versatile and knowledgeable within most areas of clinic operations. In her current M.A. position, Alyshia serves our patients in blood draws, lab processing, injections, X-Rays, rooming of patients, collection of patient vitals, and assisting with procedures. She can also jump in to help in other positions as needed due to her strong knowledge of all areas of the clinic workflow processes.

Whether it is at the front desk or on the clinical side, Alyshia is critical part of our team. When not in the office, Alyshia enjoys spending time with her family.

Robin Crouch

Robin is a graduate of Oklahoma State University with a B.S. in Psychology. She spent 8 years in the corporate HR world at BOK Financial before returning to Chandler to help Dr. Crouch open his practice in 2007. Robin has 24 years of experience in Human Resources and 16 years experience in healthcare and small business management. She is the Business Manager/Office Manager and manages all business aspects of the practice. Her office hours are part-time to allow for the full-time job of raising the family. Outside of the office, Robin enjoys supporting her children in their activities, family road trips, camping and hiking, as well as volunteering and serving in different ways in the the Chandler community that she loves.

She and Dr. Crouch have been married for 23 years and have two young daughters, Tatum & Chloe.


Lacey joined us in October of 2011 as a Medical Office Coordinator. She is a vital part of the team working at the front desk serving our patients in making appointments, fielding phone calls and communicating patient messages, checking patients in and out of their appointments, new patient registration, billing/account inquiries, medical records preparation, general customer service, and all aspects of the front desk operation. She is one of the friendly faces that welcomes you when you come in the front door and one of the first people you talk to when you call the clinic. Lacey keeps us organized and is highly skilled working our insurance claims and general process improvement to make the office run more efficiently.

When not in the office, Lacey enjoys spending time with her husband and children.


Raegan is the newest member of our team. She joined us in the position of Medical Office Coordinator in October 2023.

Raegan is a recent graduate of Chandler High School and a graduate of Gordon Cooper Technology Health Careers Program where she earned her C.N.A. She is a lifelong resident of Chandler. When not in the office, Raegan enjoys spending time with her close-knit family.

Raegan has proven to be a natural at good customer service. Her friendliness and positive energy radiates from the front desk at each patient interaction. We look forward to working with Raegan in our continued mission of providing excellent, comprehensive primary care (and beyond) to our community!
